• Given the unprecedented closure of other allergy offices in our region and the volume of students needing school forms, when possible, we suggest having school forms filled out by the student's primary care provider (pediatrician or family medicine provider)​​​​

 

• For current patients, school forms for food allergies or asthma can be completed at scheduled appointments, along with accompanying prescriptions.

• For new patients, given current patient volumes, we suggest forms and refills from your primary care provider

  

• Please be aware that Form Requests outside of scheduled appointments will incur a form fee.   

​

• All established Medicaid (Molina) recipients are required to schedule appointments for School Form Request, due to insurance requirements.  The student must be present for this appointment.

• Please be aware that Form Requests outside of scheduled appointments will incur a form fee.  Payment may be made over the phone or in person.  The office/provider will complete the form within 5 business days of receiving the school forms.

• Completed forms will be submitted to the patient portal or available for pickup. 

 

• The fee for completion of required school forms outside of an office visit is $25.  Additional forms for sports, camps, or afterschool activities will incur additional form fees.
